How to Complete a Subsequent Care Plan or Condition Assessment

In addition to ThoroughCare's initial care plan assessment, users now have the ability to begin a new subsequent care plan along with individual condition assessments. The subsequent care plan allows users to begin/open a new patient care plan with the past care plan's answers included. This makes it easier for users to make small changes to the care plan without completely starting over.

The individual condition assessments are helpful when a new manageable condition is added to the patient's care plan after the initial care plan is already finalized. Instead of having to complete yet another assessment on all of the patient's conditions, users now have the ability to complete an assessment for a single condition.

Note: Both the subsequent care plan and condition assessment will be explained in this article.

Navigate to the Patient's View Page:

Search for the target patient by using the search bar at the top of your ThoroughCare dashboard. Enter patient First Name, Last Name, or EHR ID.

Once the search page loads, click the target patient's Name, EHR ID, or "View" button located underneath the "Actions" column.

Beginning a Condition Assessment:

Once on the patient's view page, notice under the "Conditions" tab there is an "Assessment" column with "Start New" buttons visible.

Note: Make sure there is not a care plan assessment already opened, if so, you will not see an option to "Start New"

If a user wishes to start a new condition assessment click the "Start New" button located to the right of the target condition.

Once the assessment is loaded, notice that the title of the assessment is specific to the condition that was chosen.

Note: Only the condition specific questions will be included in this assessment, this makes the condition assessment rather short compared to the normal care plan assessment.

Once a single question is answered or changed the user will see a "Finalize" button appear underneath the listed assessment sections to the left of the question they are viewing. If clicked, the "Finalize" button will automatically complete/finalize the current assessment.

If a user happens to select the "Finish Later" button, in the upper right hand corner of the assessment screen, they will not be able to begin another assessment until that one has been closed/finalized.

Beginning a Subsequent Care Plan Assessment:

Users can begin a new assessment in two ways. The first involves, navigating to the patient "Programs" tab with the correct program selected under the "Programs" tab. In this example we will be completing a new care plan for a CCM patient. Click the "New" button located underneath the "Care Plan" column to begin a new assessment.

Note: Users can also begin new care plans by navigating to the patient's program page, as shown below, and clicking the "New" button at the top of the "Care Plan Review" tab.

Once a new care plan is started, the user will be asked to confirm the patient's conditions that will be included in the new assessment. Click "Save & Start Assessment" if the listed conditions are correct.

Once the assessment populates notice that the previous answers to the care plan are entered into the subsequent care plan. This allows our users to keep track of their previous answers in order to make completing a new care plan as painless as possible.

Whenever the care plan is completed or the necessary changes are made click the "Finalize" button.

Note: The "Finalize" button only appears once a single answer has been added or changed.
