How to Mark a Patient as Non-Eligible or Void / Cancel TCM

If a TCM is accidently started for a patient, or a patient becomes ineligible for their TCM, users will now have the ability to "void" an already begun TCM. To learn more on how to use the new Cancel TCM feature continue through the following article.

Navigate to the Patient's TCM Page:

First, begin by traveling to the patient's TCM program page. One can do this by clicking the "View" button located under the "Actions" column on the "TCM Program" tab of the patient view page (as shown below).

Once on the TCM page, there should be an orange "TCM in Progress" dropdown in the upper right hand corner. Click on this dropdown to change the status of the patient's TCM.

Once clicked, a "Change TCM Status" option should appear. Select "Change TCM Status" to bring up the "Change TCM Status" pop-up.

Void / Cancel TCM Status Option:

The "Change TCM Status" pop-up should open in the middle of the page. Notice the different status options that can be chosen. Let's take a look at the "Void / Cancel" option.

If a user would like to void/cancel the patient's TCM, ThoroughCare will need a reason why the TCM is being voided. Click within the "Select Reason" dropdown to select a reason for cancellation. Select the reason that best fits the change in status.

The patient information on the TCM Worklist will be changed to “Voided/Canceled” under TCM Status. Clicking the View button will allow a provider to still see the TCM entry, however making any changes to the entry is disabled while the TCM is Voided/Canceled.

If a provider or care manager attempts to make changes to a Voided/Canceled TCM entry then they will be met with the error message below and any attempted changes will not be saved.

Details of a voided/canceled TCM can be found under the Transitions tab in a patient’s file. Clicking the Summary button on the very right of an entry will bring up the Transition Summary modal displaying information concerning the patient’s admission, discharge, and TCM cancellation. The selected reason for voiding/canceling the TCM will be found at the bottom under the Cancel Summary section.

Please note that the reason behind a patient’s canceled TCM can be changed after the original void/cancel action. Users may select another Void/Cancel reason and then click save to update the patient file.

Users may also select the Non-Eligible option and then choose the reason that the patient is not-eligible for the TCM program. If a user changes a patient’s TCM status to Non-Eligible then the change will be reflected under the Status column in the Transitions tab.

Non-Eligible TCM Status Option:

If the "Non-Eligible" status option is chosen, a user will have the choice of five different cancelation reasons:

  • Outpatient procedure less than 6 hours

  • 30 day readmission

  • Late Notification

  • Discharged to Skilled Nursing Facility

  • Other

Click to select the reason that best fits the patient's situation.

Note: A user may only select a single status change and a single reasoning from this pop-up.

Finalizing Status Change:

Once the status change is complete/selected, click the "Save" button in the bottom right hand corner of the pop-up.

If the status change is successful the user should be routed back to the patient's view page. Check the "Status" shown for the patient's TCM to verify the status has been changed (shown in below screenshot).
