Add a Customized Program Condition

Enter Customer Specific Program Conditions Instead of Choosing Only From the Default List

As a customer admin, users have the ability to add their own customer specific program conditions to ThoroughCare.

Note: Only customer admins have the ability to add Custom Conditions.

Navigate to the Customers Page:

From the dashboard, navigate to the "hamburger" menu and click the drop down arrow next to the "Management" tab. Select the "Conditions" option.

When the "Manage Conditions" page appears, notice the "Practice Specific Conditions" table at the top of the page. This will be where any practice conditions will be populated right now it is blank.

Viewing Global Conditions:

Underneath that, the user will see ThoroughCare's list of current "Global Conditions" along with the programs patients with that condition may qualify for.

Adding a New Practice Condition:

To add a chronic condition, click "Create New Condition" on the top right-hand side of the page. 

An "Edit Condition" pop-up will appear. Here the user will enter the name of the new condition in the text box, then click the check boxes to select which programs the condition is to be added to.

Whenever the "Edit Condition" pop-up is completed click "Save".

Note: If you click out of the pop-up without saving, the new condition will not populate.

Once added, the condition will now be populated in the "Practice Specific Conditions" table.

Note: ThoroughCare tracts the amount of patients that have been assigned the practice specific condition under the "Number of Patients" column.

Editing a Created Condition:

Admin users can edit the condition name or delete the condition that has been added, at any time. To do this, select either the "Edit" or "Delete" options under the "Actions" column.

Note: Be careful, deleting practice specific conditions will erase them from the patient's chart which may cause patients to become illegible for programs such as CCM.

The practice specific conditions will be added to the list of ThoroughCare conditions when a user attempts to add a condition to a patient's chart. If you would like to learn how to add conditions to patient's charts click here.
