Create AWV Reports and Make Changes to Completed AWV's

Before or after the AWV is Finalized, users have the ability to print a report encompassing the completed AWV for both the patient and provider. This article will cover how a user can generate an AWV report from the patient's "Finalize AWV" page.

Navigate to the AWV Exam Page:

Search for the patient on the dashboard by First Name, Last Name or Patient EHR ID.

Once the patient search page loads click the the patient's EHR ID, patient name, or "View" button located underneath the "Actions" column.

On the patient view page, select the "Programs" tab and view the "AWV" option.

Under the "Current Annual Wellness Visit" section, click the "Open AWV Page" button highlighted below.

Note: If the patient has already completed their AWV, they will not have an "Open AWV Page" button, so a user will have to click the blue AWV link at the bottom of the patient's "AWV" tab.

Once the AWV page loads, make sure you are viewing the "Finalize Wellness Visit" section of the AWV, then scroll down the page until you see a section labeled "Provide Personalized Screening Plan to Patient".

Once the report section is located, take a look at the two report options that ThoroughCare offers.

Select Report Options Pop-up:

If the user would like to open a print preview of the AWV report in their chrome browser, one would need to click the blue "Print / Preview Summary" button. If a user wishes to create a report that is directly downloaded into the user's browser, one would select the blue "Download PDF Document" button.

Note: For this example we will be using the "Print / Preview Summary" option.

Once the "Report Options" pop-up appears take a look at the possible items that can be included in the report.

Define Report Type and Data to be Included:

AWV reports can be created in two formats, a "Patient Copy" or a "Provider Copy". Listed below are the differences between the two.

Provider Copy may Include:

  • Risk Factors

  • HRA Answers, Risks, Considerations, and Notes

  • Mini-Cog Results

Patient Copy may Include:

  • Healthy Eating Plan

  • Benefits of Exercise

  • High Cholesterol Levels

  • Smoking

  • All About Blood Pressure

  • Depression

  • Body Mass Index (BMI)

  • Strength and Balance Exercises

  • Preventing Falls

  • Type 2 Diabetes

  • Aspirin Therapy

Click to select which report format should be generated, "Patient Copy" should be pre-selected upon opening the "Report Options" pop-up.

All optional materials should be pre-selected. Deselect the blue checkboxes to the left of the items that are meant to be left out of the new report.

Note: Users have the ability to "Select All" or "Select None" of the items listed at the bottom of the pop-up by clicking the buttons highlighted below.

Generating the Report:

Once all of the desired report options are selected click the green "Print Preview" or "Download PDF" button located in the upper right hand corner of the pop-up.

While the report is generating the pop-up will show a blue "Generating document. Please wait" message, as shown in the screenshot below.

Note: Give the report a minute or so to generate

Once the report generates, it will open up in a new tab within the user's browser, as shown below, or as a download file that will appear in the bottom right hand corner of the user's browser.
