How to Complete the Review Patient Reported History Portion of the Annual Wellness Visit

Navigate to the Patient View Page:

Begin by searching for the patient on the dashboard by First Name, Last Name or Patient EHR ID. 

To navigate to the Patient Care Page, select the patient's name, EHR ID, or click "View" located under the "Actions" column.

Once on the Patient Care Page, click the "AWV" tab located under the patient notes. 

Once on the Patient's AWV landing page, click the "Open AWV Page" that is visible under the "Current Annual Wellness Visit" section.

Reviewing Patient Reported History:

On the patient's AWV Exam page, click the empty "Review Patient Reported History" bubble or the green "Start the Next Step" button at the bottom of the page.

Note: The patient's "Reported History" is gathered from the questionnaire which must be completed before beginning the "Review Patient Reported History" section.

Once the page loads, scroll down to view the "Review Patient Reported History" section which will appear at the bottom of the page.

Take a second to look over the "Review Patient History Section".

Note: Everything showing on the right side of the "Review Patient reported History" section is pulled from the patient's completed HRA answers.

Adding Notes:

Review the patient history listed on the left side of the page. If a user would like to add a note to the patient reported history click the text box located directly to the right of history box.

Reviewing Reported Screenings, Vaccines, and Tests:

Near the bottom of the page users will notice a "Screening / Vaccine / Test" section. Here, users can view and change the screening and vaccine information that was entered in the HRA.

Click a date box in the "Actual Last Date" section to choose the date that the test/vaccine was administered. If there is no record of the date or the patient does not remember the date select the "No Record" option.

If a user would need to mark all dates as "No Record", click the "Select All" checkbox located to the right of the "Actual Last Date" column header.

If there are any notes that need added to the "Screening / Vaccine / Test" section click the "Other notes about patient vaccines / screenings / tests" text box and begin typing.

Completing the Review Patient Reported History Section:

Whenever the section is completed and all necessary notes are added click the "Mark as reviewed and complete the section" button at the very bottom of the page.

If the section is successfully completed the user will see a green checkmark now populating the bubble above the "Review Patient Reported History" item.
