How to Complete the PPPS / Counseling / Referrals Portion of the Annual Wellness Visit

Navigate to the AWV Exam Page:

Search for the patient on the dashboard by First Name, Last Name or Patient EHR ID.

Once the patient search page loads click the the patient's EHR ID, patient name, or "View" button located underneath the "Actions" column.

On the patient view page, select the "Programs" tab and view the "AWV" option.

Under the "Current Annual Wellness Visit" section, click the "Open AWV Page" button highlighted below.

Beginning the PPPS / Counseling / Referrals:

Once on the patient's AWV exam page, click the empty bubble located above the "PPPS / Counseling / Referrals" section. Users can also select the green "Start The Next Step" button located at the bottom of the page to navigate to the next section of the patient's AWV.

When the section loads scroll to the bottom of the page to view the patient's "PPPS / Counseling / Referrals".

Reviewing the "PPPS / Counseling / Exams" Sections:

Notice the first part of the "PPPS/ Counseling/ Referrals" is labeled "PCV13 Pneumococcal shot".

Note: The "PPPS / Counseling / Exams" section has been broken up into different sections similar to the "Review Patient Reported History" and "Review Assessment for Risk" portions of the AWV.

Notice the first column of each individual "PPPS / Counseling / Referrals" section lists the name of the target preventative screening.

Reviewing Patient Reported:

Notice if there is anything populated in the "Patient Reported" column. If it's empty, this means a date was never provided.

Note: If the patient provided a date for any of the tests/screenings listed on the "Review Patient Reported History" section that date will appear under the "Patient Reported" column. The same goes for tests marked as "No Record".

Reviewing Patient Recommendations:

The suggested time frame and circumstances in which a patient should receive these particular tests is outlined in the next column labeled "Recommendation".

Note: Recommendations are provided based off of the patient answers recorded during the AWV assessment.

Adding to a Patient's Preventative Schedule:

The next column of an exam focuses on documenting and scheduled referrals for the screenings/tests the patient should have completed.

Click the first provided drop-down bar to select if the test is an "Immediate" need, click the second to choose when the future exam is "Scheduled", and click the third to select the "Frequency" in which the patient will receive the screening/exam.

Completing/Hiding a "PPPS / Counseling / Exams" Section:

Once a user has completed reviewing a patient's screening/test section, they have the ability to "hide" the reviewed portion by clicking the "Arrow" icon in the upper right hand corner of the screening's box.

Completing the PPPS / Counseling / Exams:

Whenever the preventative schedule is filled out to the best of the Care Managers abilities, the "PPPS / Counseling / Referrals" section can be marked as reviewed by clicking the "Mark as reviewed and complete the section" button located at the bottom of the page.

When the "PPPS / Counseling / Referrals" portion is completed, the bubble above the "PPPS / Counseling / Referrals" section will be populated with a green checkmark.
