Updating a Care Plan Item

While on the Problems tab, the problems list will populate with flags that identify the problem status and the type of problem (Condition or Barrier).

Once a problem has a finalized care plan, the problem status changes from Unmanaged to Managing. You may then select the managed problem to add additional information.

Once you select a managed problem, the following sections will appear for you to review with your patient and/or add additional information: Outcomes, Symptoms, Long Term Goals, SMART Goals, Interventions, Barriers and Problem Notes.

Each section will contain previously applied care plan items (Outcomes, Symptoms, Long Term Goals, SMART Goals, Interventions, and Barriers) and an entry box to add new care plan items.

Review each of the problem's care plan items with your patient. Select the blue text to open up each of the care plan items.

Items that have gray text are either achieved, closed, or resolved.

The care plan item's modal will pop up providing the opportunity to update the Status, apply a date to the update, write any relevant notes, review any previous notes, and provide a motivation (for Long Term Goals and SMART Goals only).

To change the status of a care plan item select the status from the Status dropdown.

The new status will be reflected in the previous notes section after selecting the Save Button.

To add a note to the care plan item, type the note in the Notes section of the modal and save to apply the note to the care plan item.

Users may edit a note they entered by clicking the Addendum button and then clicking either the blue Save button or the red Delete to remove the note.

​Note: Admin users may make addendums on any notes entered by users within their practice.

Adding a motivation to a Long Term Goal or SMART Goal can be done by selecting a number on the motivation scale.

Adding a new Care Plan Item

If a new care plan item is needed, simply begin typing in the corresponding care plan item's New dropdown.

A list of pre-populated, standard best practice responses will display for you to choose from. Select the correct response and it will then be added to the problem.

Problem Notes

It may be helpful to include broad, condition-specific information under the Problem Notes section, such as:

  • Date of diagnosis

  • Doctor managing this condition

  • Related symptoms

  • Medications

  • Lifestyle modifications implemented

  • Complications

Example: Patient was diagnosed with HTN in 2017 following a heart attack where 2 stents were placed. Dr. John Rogers, cardiologist and PCP manages this condition. Currently, patient is seeing cardiologist every 6 months. Patient reports he is asymptomatic. Currently taking losartan and propranolol to manage the condition. Patient occasionally checks blood pressure (apx 4x a month) with the last reading being 146/88.

Note: It may be helpful to use the Problem Notes section as a condition overview to be read as a quick refresher.

Changing the status of a Managing Problem

If the status of the problem needs to be change from the Managing status, select the appropriate status from the Status dropdown and select Save. The new status will be applied to the problem.
