Managing patient goals

Goals Tab

To manage goals for a patient, select the Goals tab from an Initial or Ongoing Monthly Workflow then select the goal you would like to manage with your patient. You can filter the Goals list by Problem and/or Goal Status or add a new goal by selecting the + icon in the top right corner of the Goals tab.

Updating an Existing Goal

The Goals tab will list all goals assigned to your patient. Each goal will be listed on this tab with the Problem it is assigned to and the Goal Status.

Select the goal you would like to update with your patient. The goals list now moves to the left side of the Goals tab and the selected goal is now open for you to update.

The following fields will appear for the selected goal: Status, Date, Notes, Previous Updates and Scale. Select a Status from the dropdown box.

Today's date will default in the Date box and can be updated by clicking on the date.

Add your Goal Notes. For the Motivation Scale, you can update the patient's readiness to change. The patient can select a number 1-10 to indicate their motivation to address this specific goal.

This is especially helpful to the care manager, who is making a concerted effort to engage the patient. By focusing on goals the patient is more motivated to achieve, you increase the likelihood of the patient making positive changes.

Once the goal is updated, click Save in the bottom right corner.

You will now see your goal update appear in the Previous Updates box with date stamp and user name. Previous Updates are read-only and cannot be edited.

Adding a New Goal

You may add additional goals at any time by clicking the + icon in the upper right corner of the Goals tab.

An Add Goal modal will appear to document new goals with your patient. Here you may add a Goal or a SMART Goal by selecting the appropriate tab.

To add a goal to the Goal dropdown box, simply click inside the dropdown box and select the patient’s goal.

If your selection is not listed in the dropdown, begin typing your goal; a list of pre-populated, standard best practice responses will display for you to choose from.

Then select a Goal Status.

Assign to Current Problem (the patient's current problems will be listed in the dropdown for you to select).

Once the new goal is complete, select Save in the bottom right corner.

The new goal will now be listed on the Goals tab with the selected Goal Status and Assigned Problem.
