To manage interventions for a patient, select the Intervention tab from an Initial or Ongoing Monthly Workflow then select the intervention you would like to manage with your patient. You can filter the Interventions list by Problem, Action and/or Intervention Status or add a new intervention by selecting the + icon in the top right corner of the Intervention tab.

Updating an Existing Intervention
The Interventions tab will list all interventions assigned to your patient. Each intervention will be listed on this tab with the Problem it is assigned to and the Intervention Status.
Select the intervention you would like to update with your patient. The intervention list now moves to the left side of the Interventions tab and the selected intervention is now open for you to update.
The following fields will appear for the selected intervention: Problem, Status, Date, Notes, and Previous Updates.
Select a Status from the dropdown menu.
Today's date will default in the Date box and can be updated by clicking on the date.
Add your Intervention Notes.
Once the intervention is updated, click Save in the bottom right corner.
You will now see your intervention update appear in the Previous Updates box with date stamp and user name.
Note: If the status of the intervention was updated, it will appear in the Previous Updates section as old status {arrow} new status (ex: Pending -> In Progress).
If a user needs to make an edit to a note they entered, then they can click the "Addendum" button to the right of the "Previous Updates" header. After making edits, the user can click the blue save button or click the red delete button to remove the note.
Note: Admins may make addendums on any notes entered by users within their practice.
Adding a New Intervention
You may add additional CCM interventions at any time by clicking the + icon in the upper right corner of the Interventions tab.
An Add Intervention modal will appear to document new interventions with your patient. You can choose from one of the three types of interventions to add: Care Manager Actions, Patient Actions, Services and Programs.
All 3 Intervention tabs will contain an Intervention dropdown box, Assign to Current Problem dropdown menu and Intervention Status dropdown menu.
To add an intervention to the Intervention dropdown, simply click inside the box and select the patient’s intervention.
If your selection is not listed in the dropdown, begin typing your intervention; a list of pre-populated, standard best practice responses will display for you to choose from.
Assign to Current Problem (the patient's current problems will be listed in the dropdown for you to select).
Select a Goal Status (In Progress, Resolved, Closed, or Closed: Import Error).
Once the new intervention is complete, select Save in the bottom right corner.
The new intervention will now be listed on the Interventions tab with the selected Intervention Status and Assigned Problem. This is an effective way to add interventions after the care plan has been created; especially if you limit initial interventions to not overwhelm the patient.
