The Summary of Care tab provides a comprehensive view of a patient's Problems, Goals, Barriers, Interventions, Patient Vitals and Patient Notes. You may use the Summary of Care tab as a quick overview of a patient's care plan or add additional information in each section, if needed.

Select the Problems tab to view a patient's problem list. You may Include Cancelled/Removed Problems by selecting the checkbox in the upper right corner or add a problem by selecting the blue + also in the upper right corner.

The problems list will populate with flags that identify the problem status (Unmanaged, Managed, Resolved or Removed) and the type of problem (Condition or Barrier). Once a problem has a finalized care plan, the problem status changes from Unmanaged to Managing. Select a problem to view or add an update.

Select the Goals tab for a comprehensive view of the patient's goals. You can filter the Goals list by Problem and/or Goal Status or add a new goal by selecting the + icon in the top right corner of the Goals tab.

The patient's goals will be listed on this tab with the Problem it is assigned to and the Goal Status (Pending, In Progress, Ongoing, Achieved, Closed or Legacy). Select a goal to view or add an update.

Select the Barriers tab for a comprehensive view of the patient's barriers. You can filter the Barriers list by Problem and/or Barrier Status or add a new barrier by selecting the + icon in the top right corner of the Barriers tab.

The patient's barriers will be listed on this tab with the Problem it is assigned to and the Barrier Status (Pending, In Progress, Ongoing, Achieved, Closed or Legacy). Select a barrier to view or add an update.

Select the Interventions for a comprehensive view of the patient’s interventions. You can filter the Interventions list by Problem, Action and/or Intervention Status or add a new intervention by selecting the + icon in the top right corner of the Intervention tab.

The patient's interventions will be listed on this tab with the Problem it is assigned to and the Intervention Status (Pending, In Progress, Resolved or Closed). Select an intervention to view or add an update.

Select the Patient Vitals tab to view a history of the patient's vital readings. The patient's vitals will be listed on this tab with Date Recorded, Value and Average. Select Details in a specific vital to view or add an update.

Select the Patient Notes tab to view a history of the patient’s notes. Overall Patient Notes and Overall Provider Notes will be listed on this tab with the date notes were added. Select the date of the note to view additional information including the care manager or provider who added the note. Select the + icon to add additional notes.
