Overview

The new Care Management Header, located within the Care Management tab, is a preview of ThoroughCare's evolution of the care management workflow, designed to provide immediate clinical context at a glance. To ensure a clean user interface and preserve vertical screen real estate, this section will be initially collapsed by default. Users can simply click to expand or collapse as needed.

By moving toward this patient-centric view, ThoroughCare allows your staff to manage the patient, not just the program. Whether you are checking enrollment eligibility, tracking clinical goals, or preparing for an upcoming call, this header acts as your command center for patient coordination.

Program Status

This section identifies all programs available to your practice, displaying the patient’s current enrollment status and the total number of conditions being managed. A condition count shown in red indicates the patient does not currently meet the minimum clinical requirements for that program; for example, a CCM patient with only one condition would appear in red as the program requires at least two. Additionally, users can click the enrollment status to open a modal and quickly enroll or inactivate the patient within the selected program.

Note: This new section will allow for directly enrolling patients into the APCM program, APCM billing logic will still look for the flags added to the patient at this time.

Care Plan Progress

Measuring the efficacy of a care plan is vital for successful outcomes. The Care Plan Progress section transforms complex clinical data into a visual donut chart, allowing care managers to evaluate a patient’s journey toward wellness instantly.

  • Key Metrics: The chart breaks down the care plan into four distinct categories:

    • Completed: Goals, barriers, or interventions that have been successfully resolved.

    • Goals In Progress

    • Barriers In Progress

    • Interventions In Progress

  • This visualization helps clinicians identify at a glance if a care plan is "stagnant" (many barriers/interventions in progress) or "thriving" (a high percentage of completed items). It serves as a powerful tool for self-auditing the impact of your care interventions.

Patient Interactions

The Patient Interactions section ensures that your team is always aware of the patient’s communication history and future schedule. This section is divided into two primary focus areas:

  • Last Successful Interaction: This displays the details of the most recent interaction that was documented as "successful." Knowing exactly when you last had a meaningful conversation with the patient is essential for maintaining the clinical cadence required for various CMS programs.

  • Upcoming Interactions: To help with forward-looking planning, this sub-section lists up to 5 interactions scheduled for the future. This prevents duplicate scheduling and ensures that the care team is prepared for the next touchpoint, whether it is a routine check-in or a specialized follow-up.
