How to Complete the Mini-Cog Portion of the Annual Wellness Visit

Navigate to the AWV Exam Page:

Search for the patient on the dashboard by First Name, Last Name or Patient EHR ID.

Once the patient search page loads click the the patient's EHR ID, patient name, or "View" button located underneath the "Actions" column.

On the patient view page, select the "Programs" tab and view the "AWV" option.

Under the "Current Annual Wellness Visit" section, click the "Open AWV Page" button highlighted below.

Beginning the Cognitive Assessment:

Once on the patient's AWV exam page, click the empty bubble located above the "Cognitive Assessment" section. Users can also select the green "Start The Next Step" button located at the bottom of the page to navigate to the next section of the patient's AWV.

The section will load below the patient information, scroll down the page to view the "Cognitive Assessment" section.

Three Word Registration:

The first part of the "Mini-Cog" will be listed underneath "Step 1". Look at the patient and repeat the words that are listed within "Step 1". Keep a tab on if the patient was able to repeat the words said within three attempts.

Clock Drawing:

When the patient is ready for "Step 2", proceed to ask them to draw a clock's face. Record if the patient was able to accurately draw a clock.

Note: ThoroughCare provides a clock template if the user needs one to complete the AWV. Click the "Print Blank Clock Circle" button to download a printable template for the patient.

Three Word Recall:

Now ask the patient if they are able to recall the three words that were read to them in "Step 1".

Scoring the Cognitive Assessment:

Once the patient has completed the tests, record tests scores at the bottom of the page underneath the "Scoring" section.

Once the patient's answers are recorded, ThoroughCare will calculate a "Total Score". If the score receives a green message that means the patient has passed the Mini-cog.

Note: If a patient still has an orange message even after scoring, they are eligible for Dementia screening.

Adding Additional Notes:

If any notes are need click the "Additional Notes" box at the bottom of the screen to begin typing.

Whenever the section is completed click the "Mark as reviewed and complete the section" button at the bottom of the page.

When the "Cognitive Assessment" is completed, the bubble above the "Cognitive Assessment" section will be populated with a green checkmark.
