Navigating to the AWV Landing Page:

Search for the patient in the dashboard by First Name, Last Name or Patient EHR ID.

Once the patient search page loads, click the the patient's EHR ID, patient name, or "View" button located underneath the "Actions" column.

On the patient view page, select the "Programs" tab and view the "AWV" option.

Under the "Current Annual Wellness Visit" section, click the "Open AWV Page" button highlighted below.

Beginning the Review Assessment for Risk:

Once on the patient's AWV exam page, click the empty bubble located above the "Review Assessment for Risk" section. Users can also select the green "Start The Next Step" button located at the bottom of the page to navigate to the next section of the patient's AWV.

Once clicked, scroll to the bottom of the web page to view the "Review Assessment for Risk" section.

Viewing Risk Factors Identified:

Under the "Possible Risk Factor Identified" tab users can review the patient's possible risk factors for the answers to the HRA provided by the patient. The box to the right, in the screenshot below, highlighted in red, gives the user suggestions to help with the risk that was identified.

Viewing a Patient's Assessment Responses:

The next tab labeled "Assessment Responses" just lists the HRA responses that were collected in the assessment.

Adding Notes to a Review Section:

If a user needs to add any notes to a section, navigate to the "Notes" tab and click the text box that appears to begin typing.

Documenting Actions Taken to Manage Risk:

If the user decides to take action to manage the patient's risk, the AWV provides check boxes to mark what action was taken per each section. These options are:

Counseled Patient for Risk
Will Provide Literature / Handouts
Make Patient Referral
Educated Patient on This Topic

If literature was provided or a referral was made, users can specify these by typing in the text boxes provided in the bottom right hand corner of the target section that is focused on.

Hiding a Review Assessment for Risk Section:

Once a user is finished reviewing a particular section, that portion of the "Review Assessment for Risk" can be collapsed by clicking the "Arrow" icon located in the upper right hand corner of the target section.

Once clicked, notice the section has been hidden from view.

Completing the Review Assessment for Risk:

Continue through the rest of the sections, after all potions are complete click the "Mark as reviewed and complete the section".

Once the section is marked as completed, notice the "Review Assessment for Risk" bubble will now be populated with a green checkmark, this lets the user know that this section of the AWV has been completed.
