Navigate to the AWV Exam Page:
Search for the patient on the dashboard by First Name, Last Name or Patient EHR ID.
Once the patient search page loads click the the patient's EHR ID, patient name, or "View" button located underneath the "Actions" column.
On the patient view page, select the "Programs" tab and view the "AWV" option.
Under the "Current Annual Wellness Visit" section, click the "Open AWV Page" button highlighted below.

Once on the patient's AWV exam page, click the empty bubble located above the "Finalize Wellness Visit" section. Users can also select the green "Start The Next Step" button located at the bottom of the page to navigate to the next section of the patient's AWV.
The section will load below the patient information, scroll down the page to view the "Finalize Wellness Visit" section.
Enrolling Patient in CCM:
The first section under "Finalize Wellness Visit" will ask the user if the patient would like to participate in the CCM program (if applicable).
Note: If the patient does not want to enroll in the CCM program skip down the "Printing the AWV Summary for the Patient or Provider" section of this article.
To enroll a patient in CCM click the blue "Change Status" button and and enrollment pop-up should appear.
To enroll the patient in the CCM program, click the "Change enrollment status to:" drop-down and select the "Active".
Once the patient is marked as "Active" the "CCM Enrollment Status " pop-up will extend to document patient enrollment. Complete the enrollment form with the patient as usual.
When the form is completed click the "Save" button at the bottom of the pop-up.
Once the patient is enrolled, the "Evaluate Chronic Care Management" section will populate the date in which enrollment was completed (shown below).
Printing the AWV Summary for the Patient or Provider:
The next section underneath "Finalize Wellness Visit" is designated for printing the AWV summary.
If the user wishes to open a google chrome preview for the AWV summary they would choose the "Print/ Preview Summary" button, but if the user would like to download the document they would click the "Download PDF Document" button.

If the user has printed and given the summary to the patient make sure to check the "Mark that Patient Received a Copy" check box.
Note: This needs to be checked to finalize the patient's Annual Wellness Visit.
Provider Sign off and Finalize AWV:
Have the provider sign off on the patient's AWV by clicking the check box to the left of "I sign off that this AWV had been completed". Next, make sure the "Date Completed" is correct to the date that the AWV had been finished, today's date will automatically be populated.
When completely finished click the large green "Finalize and Bill" button, located t the bottom of the page.
If the AWV is successfully completed, ThoroughCare will notify the user in the upper right hand corner of the screen.
Note: The AWV "Status" from the patient view page will reflect the date that was chosen in the step above.
If the user wishes to open the AWV after completion, click the blue link to the completed annual wellness visit shown at the bottom of the "AWV" program tab, as shown in the screenshot below. Users cannot edit the AWV once it is marked as completed.
Note: The AWV will always be printable from the "Finalize Wellness Visit" section of the patient's completed AWV.
Once the AWV is completed, the "Finalize Wellness Visit" bubble will be populated with a green checkmark.
