Utilizing Honor My Decisions for Advance Care Planning (ACP)

ThoroughCare's partnership with Honor My Decisions allows for more comprehensive Advance Care Planning (ACP).

As part of an Annual Wellness Visit (AWV), providers are instructed to provide ACP services at the discretion of the patient. ACP is instrumental in ensuring your patients have a voice in their future care needs. While it’s importance is unquestionable, ACP has the potential to be daunting for both the clinical staff as well as the patient. It can be an uncomfortable situation for patients to consider unfavorable situations for themselves and its also challenging for clinicians to review so many scenarios and what-ifs. Honor My Decisions (HMD) provides patients and clinicians with an easy-to-follow guide in navigating complex decisions. By facilitating the conversation through education and scenarios, HMD helps create more space for the patient to share their values and priorities with their healthcare team. When approaching the ACP conversation, it is important to note for the patient that ACP is a covered Medicare benefit and HMD can provide them with legally binding documents at no additional cost. When ACP documents are completed in HMD, the patient will have 24/7 access to those documents in addition to printing them out. This ensures your patients and their loved ones will always have access to their documents which is imperative as health care emergencies are never planned.

Benefits

The benefits of Honor My Decisions include:

  • Providing educational resources for advance directives which is a requirement to offer for AWV

  • Free, legally binding ACP documents

  • Video living wills, supplemental end-of-life values and priorities exercises to enhance advance directives

  • Access to a printable version as well as an electronic version of advance care plans which can be accessed anytime by the patient. In the case of a medical emergency and the patient does not have their physical copy, they would be able to access it electronically

Access through an AWV

During the process of completing the AWV, select "Yes" for the question "A covered benefit of Medicare is having the opportunity to discuss and create legal life planning document that reflect your wishes. Would you like to create any life-planning documents?". This will prompt the Honor My Decisions iframe when entering the Advance Care Planning portion of the AWV.

Once the Advance Care Planning section of the AWV is reached, select the "Patient consents to ACP Counseling - Begin Now" button, this will prompt the Honor My Decisions iframe.

Use the workflow within the Honor My Decisions iframe to walk through the ACP process.

Access Through ACP Tab

To access Honor My Decisions through the ACP Tab, first select the Wellness tab on the Patient show page and then select ACP.

On this Page you can select the "Conduct New ACP" button to begin a new ACP in Honor My Decisions

If an ACP is already in progress select the "Finish" button under the Actions tab to continue the in progress ACP.

ACP Completion

Once the ACP is completed, select the "End" button on the timer to begin the time logging activities.

Select the supervising provider from the drop down on the summary page of Honor My Decisions.

After reviewing the ACP activities performed select the "Submit Session" button.

Note: The ACP sessions must have a duration of at least 16 minutes to log the time and complete the session.

After completing the session the ACP can be accessed through the HMD ACP URL link, the View Action or the Report Action.
