Once patient care plans are created in ThoroughCare, they can be uploaded to AdvancedMD in a few easy steps. Here are some best practices to successfully upload patient care plans from ThroroughCare to AdvancedMD.

Individual Care Plan Upload
To upload an individual care plan to AdvancedMD, navigate to the patient's Care Management tab in ThoroughCare.
Select the blue Print / Preview icon.
The Select Report Details modal will open with multiple report options. The options checked below will be selected by default. Select all care plan options you would like added to the care plan PDF to be uploaded to AdvancedMD.
You may choose from several options on the left side of the modal to be included with your care plan PDF including Provider Review Action Items, Remote Device Monitoring Readings and Notes, and Date Ranges of the care plan.
On the right side of the Select Report Details modal, you may choose to select or deselect multiple sections of the care plan including additional Notes and Care Plan Progress Sections for Goals, Barriers and Interventions. Please follow your company's recommended care plan workflow to ensure consistent documentation is being uploaded to AdvancedMD.
Once all care plan items are checked, you may preview the care plan PDF by selecting Open Preview at the bottom of the modal. Or upload the care plan directly to AdvancedMD by selecting Upload to EHR.
An EHR confirmation box will appear in the top, right corner of the dashboard once the care plan is successfully uploaded to AdvancedMD.
To view the care plan PDF in AdvancedMD, navigate to the Patient Record Documents section.
The care plan PDF will be listed in the Documents section. The PDF name will contain the Patient Chart Number, Patient Name and Care Plan Upload Date.

Tap the PDF file to open. The Care Plan Summary Report opens with all care plan report details previously selected in ThoroughCare.
Bulk Care Plan Upload
Due to ThoroughCare's seamless integration with AdvancedMD, importing bulk care plans can be done in a few simple steps.
From your ThoroughCare Dashboard, select a worklist. For example: select Chronic Care Management to open your CCM Worklist.
You may filter your worklist to view specific patients.
You may select specific patients in your worklist or tap the Select All checkbox at the top of your patient list to select all patients.
Select Bulk Actions then Care Plan Report.
The Care Plan Report modal will open with multiple report options. Select all care plan options you would like added to each patients' care plan PDF to be uploaded to AdvancedMD. Please follow your company's recommended care plan workflow to ensure consistent documentation is being uploaded to AdvancedMD.
Once all care plan items are checked, you may generate individual care plan reports to preview by selecting Generate at the bottom of the modal. Or upload the care plans directly to AdvancedMD by selecting Upload to EHR.
Note: The upload time for bulk care plans into AdvancedMD will vary depending on the number of care plans being uploaded.
