New Statuses for Care Plan Items

Care Plan Items can be updated using new status definitions in order to better track measurable outcomes.

New Care Plan Item Statuses

Care Plan Items can be found on a patient’s chart, under Care Management, and then selecting a Care Plan Item tab such as Goals, Outcomes, Interventions, or Barriers.

Goals for Care Plan Items have been updated to have the following statuses:

  • Pending

  • In Progress

  • Closed: Met

  • Closed: Mostly Met

  • Closed: Not Met

  • Closed: Import Error

NOTE: Goals can also be met automatically due to system automation. When this occurs, the status will show ‘Closed: Met, System Automation.’


Barriers for Care Plan Items have been updated to have the following statuses:

  • In Progress

  • Resolved

  • Closed

  • Closed: Import Error

Interventions for Care Plan Items have been updated to have the following statuses:

  • In Progress

  • Resolved

  • Closed

  • Closed: Import Error

Outcomes for Care Plan Problems have been updated to have the following statuses:

  • In Progress

  • Closed

  • Ongoing

  • Closed: Import Error

Outcomes added to a problem will appear as blue hyperlinked text. Click on the text to display the edit modal for that outcome.

Smart Goals for Care Plan Items have been updated to have the following statuses:

  • In Progress

  • In Progress: On Target

  • In Progress: Not On Target

  • Achieved

  • Closed

  • Closed: Import Error

Smart Goals added to a problem will appear as blue hyperlinked text. Click on the text to display the edit modal for that Smart Goal.


Marking Problems as Resolved or Closed

When a Care Plan Problem’s status is set to close, cancel, or resolved, a modal will be displayed with a list of Care Plan Items (Goals, Smart Goals, Barriers, & Interventions) that are associated with the problem.

Each care plan item type will have a dropdown box next to it where the user can set the status.

Take note of the 'Reset All Care Plan Item Status' button that appears at the bottom of the modal and becomes enabled if 1 or more Care Plan Items have a status set to them.

Clicking the button will reset all care plan item dropdown statuses.

Once statuses are set, clicking the 'Save' button on the modal will apply the changes to all of the Care Plan Items shown.


NOTE: The save button is disabled until all Care Plan Items have a set status.

NOTE: Symptoms and Outcomes will automatically close, and they can be re-opened when the Care Plan Problem is reopened.
