TCM Worklist Overview
The TCM Worklist displays all patients associated with ThoroughCare's TCM program. Users can utilize the worklist to track and manage patient transitions from their admissions to billing, and everything in between.
Accessing the TCM Worklist
Users may access the TCM in two ways, both contained in ThoroughCare's left side panel menu.
To Access the TCM Worklist Directly
Click the hamburger menu icon located in the top left corner of the ThoroughCare screen
Click 'Programs' to display the list of all available program-related worklists
Select 'TCM'
When a user enters the TCM Worklist through this method, the Worklist will automatically display all 'Active' patient transitions or pre-set to the filters that the user last used.
To Access the TCM Worklist through the Dashboard
Click the hamburger menu icon located in the top left corner of the ThoroughCare screen
Click 'Dashboard'
Scroll down to the 'Transitional Care Management' section
Users may enter the TCM multiple ways through the Transitional Care Management Dashboard, depending on the type of information they are looking for. The following Dashboard headers contain hyperlinks that will direct users to a specifically-filtered TCM Worklist:
Transitional Care Management: By clicking on the Transitional Care Management Dashboard header, the worklist will open to all 'Active' patient transitions
TCM Opportunity: By clicking on the number of patients listed in the 'TCM Opportunity' column, the worklist will open to all patient transitions that are currently listed with the 'TCM Opportunity' status.
Note: This dashboard column and worklist filter are only available to practices integrated with Zus or ADT.
Due Today Number: By clicking on the number of patients listed in the 'Due Today' column, the worklist will open to all patient transitions that have a deadline occurring on the that current day
Anticipating Discharge Number: By clicking on the number of patients listed in the 'Anticipating Discharge' column, the worklist will open to all patient transitions that are currently listed with the 'Anticipating Discharge' status.
Awaiting Initial Contact Number: By clicking on the number of patients listed in the 'Awaiting Initial Contact' column, the worklist will open to all patient transitions that are currently listed with the 'Call Pending' status.
Awaiting Face-to-Face Number: By clicking on the number of patients listed in the 'Awaiting Face-to-Face' column, the worklist will open to all patient transitions that are currently listed with the 'Visit Pending' status.
Finished Claim Hold Number: By clicking on the number of patients listed in the 'Finished Claim Hold' column, the worklist will open to all patient transitions that are currently listed with the 'Finished Claim Hold Period' status.
Note: This dashboard column and worklist filter are only available to practices enabled for the TCM 30-Day Hold period.
Billed: By clicking on the number of patients listed in the 'Billed' column, the worklist will open to all patient transitions that are currently listed with the 'Finished Paid' status.
Deadline Passed: By clicking on the number of patients listed in the 'Deadline Passed' column, the worklist will open to all patient transitions that did not meet their next required deadline.
Filtering the TCM Worklist
Once a user lands in the TCM Worklist they can update the worklist's filters to create a list of patient transitions that are specific to the user's needs.
To remove filters, simply click the 'x' on the selected statuses.
To add filters, click on the 'Filters' button located in the top right corner of the TCM Worklist screen to access the list of available filter options.
TCM Worklist Filters
Care Manager: Filters the worklist for any TCM patient whose transition was documented by the selected Care Manager(s)
Physician: Filters the worklist for any TCM patient whose Face-to-Face Visit was performed by the selected Physician(s)
Primary Care Manager: Filters the worklist for any TCM patient who is associated with the selected Primary Care Manager(s) in their Patient Info screen
Primary Physician: Filters the worklist for any TCM patient who is associated with the selected Primary Physician(s) in their Patient Info screen
Flags: Filters the worklist for any TCM patient who is associated with the selected practice flag(s)
Language: Filters the worklist for any TCM patient who is associated with the selected language(s) in their Patient Info screen
TCM Status: Filters the worklist for patient transitions based on their current TCM status(es)
Active: Automatically filters the worklist for all 'Active' statuses
Inactive: Automatically filters the worklist for all 'Inactive' statuses
Discharge Location: Filters the worklist for any patient transitions that are associated with the selected Discharge Location(s)
Last Successful Call: Filters the worklist for any TCM patients whose last recorded successful call occurred during the selected timeframe
Call Attempts: Filters the worklist for any patient transitions that have had the selected number of initial communication attempts within 30 days of their most recent discharge
Billable: Filters the worklist for any patient transitions that are currently marked as Billable or Non-Billable
Next Deadline: Filters the worklist for any patient transitions that have a deadline occurring during the selected timeframe
Primary Insurance: Filters the worklist for any TCM patients whose primary insurance is set to the selected Primary Insurance(s)
Secondary Insurance: Filters the worklist for any TCM patients whose secondary insurance is set to the selected Secondary Insurance(s)
Customizing the TCM Worklist
The TCM Worklist can be customized to suit each user's individual preferences and needs by allowing them to set the number of results shown and how the format of their columns. Once a user customizes their worklist, ThoroughCare will remember their selected settings until they are manually updated.
Set the Number of Entries Shown
Users can set the maximum number of patient transitions that can be shown on the TCM Worklist screen at a time. If the worklist populates more than the selected number, users will need to navigate to additional screens to view the additional results.
Select the Columns that are Displayed
By clicking on the 'Column Visibility' dropdown button, users can select and de-select what columns are visible on the TCM Worklist. This enables users to only select column data that is pertinent to them and save space on their screens.
Set the Column Order
By clicking on the 'Arrange Columns' button, users can organize their TCM Worklist columns by clicking and dragging them into their preferred positions. Once the user has arranged the columns in their desired order, they can click the 'Save' button to lock the columns into place.
Additional Worklist Functions
The TCM Worklist contains additional functionality that users can utilize to streamline their workflows and tasks.
Adding Patient Flags
The TCM Worklist allows users to add flags to their patients in bulk.
To perform the 'Add Flags' bulk action:
Select one or more rows in the TCM Worklist
Click the 'Bulk Actions' dropdown button
Select 'Add Flags' to open the 'Add Flag to filtered patients' modal
Select one or more available flags
Click 'Save'
Once the bulk action is performed, the flags will automatically display on the worklist and in the patient records.
Generating the Worklist in a PDF
In the event users would like to save their current view of the TCM Worklist or print out a physical copy, they can click the 'PDF' button to generate a PDF version of the worklist.
Patient Search
The TCM Worklist also allows users to manually search for individual TCM patients with its own 'Search' functionality.
Please note that any returned search results will be dependent on the filters currently placed on the worklist. For example, if the TCM Worklist is currently filtered for 'Active' statuses and a user searches for a patient whose transition is in an 'Inactive' status, then the search will not return that patient as a result.
