ThoroughCare now integrates with the Tenovi Pillbox, one of their newest Gateway devices, enabling real-time tracking of patient medication adherence. These devices record when a patient opens and refills their pillbox, providing valuable insight into medication adherence and compliance.

The following are benefits of incorporating the Tenovi Pillbox when monitoring a patient's medication adherence:

  • Improved Care Management

    • Gain a clearer picture of patient medication routines in conjunction with patient’s recorded medications, allowing care teams to identify potential issues early and offer timely support.

  • Better Patient Outcomes

    • By tracking adherence more closely, care managers can address gaps in medication compliance, helping patients stay on track with their treatment plans.

  • Ready for the Future

    • This integration is designed to work seamlessly with ThoroughCare’s upcoming Remote Therapeutic Monitoring (RTM) program, creating even more opportunities to improve care delivery and engagement.

Along with the integration of the Tenovi Pillbox device, ThoroughCare also supports CPT code generation for RTM devices. We will cover how to set that up for Tenovi Pillbox in this article, but for more information on RTM, click the link here! For the Pillbox, CPT code generation can be set up when adding the device to the patient's page. Let's go over how to do that!

Adding the Device

Start first by reaching the "Add a new Device" modal as seen below.

Once here, select the Tenovi Pillbox from the Device Type dropdown, and enter the Gateway ID in the "Device ID" section.

When this is done, RTM will be the only option available, as the Pillbox should only be billed under the RTM program as of now. From here, users may check both of the checkboxes if they apply to them. For this example, we will select both boxes. Lastly select the most appropriate "Device Readings Code" for this patient's pillbox. We will select "Musculoskeletal System - 98977" for this example.

Once all the information is filled out appropriately, select the blue "Save" button.

Now that the Pillbox is added to the patient's page, the table from the beginning of this article should now appear under the "Medications" accordion within the "Clinical Data" tab.

Table Details

As seen above, the table contains three columns. Let's discuss what each column represents! The first represents the pillbox event that occurred, allowing users to get a clear idea of what their patient is doing with the device. These events can be the "opened compartment", which are labeled in alignment with the pillbox compartments shown on the device, or a pillbox refill, which is shown as "Refill".

The second column indicates when the event indicated by the first column actually occurred and is color coded based on if the event occurred at the correct time. See the image below how the green text correlates correctly to the event Day and Time, but the red text does not.

Finally, the last column on the right provides the user with the ability to remove an event, simply by clicking the "X".

Notifications

Users should be aware that ThoroughCare supports notifications for their patients' pillbox devices, so that users can be aware of their patients missing medication events! To turn on notifications for this device, first ensure that your user profile has them enabled! Head to your name in the top right corner of the screen and select "Settings"

On the next page, scroll down to the "Readings" section and make sure the "Medication Adherence" checkboxes are checked for whichever notification types you prefer!

Once this is done, head back to the patient's Medication Adherence table. Find the "Daily Required Medication Events" option at the bottom left corner of the table. Select the dropdown and view the options available.

Select the option that best suits your notification preferences

  1. None: Pillbox notifications are disabled

  2. Once per Day (AM or PM): User will be notified if the patient does not use their Tenovi Pillbox at all for a given day. (Events do not have to be "green")

  3. Twice per Day (AM and PM): User will be notified if the patient does not user their Tenovi Pillbox at least twice in a given day. (Events do not have to be "green")

Once the preference is set, then the user can start receiving their desired notifications for their patients.

Billing Information

Once the patient starts taking readings, users will see that the appropriate billing codes will appear under the patient's "Billing" tab. Just to note, CMS guidelines require at least 16 days with readings to qualify for setup and monitoring codes.

Below is what the claims will look like in ThoroughCare once 16 days of readings are reached with the Pillbox. One setup claim, which can only be billed once per patient, and one Musculoskeletal System code, which can be billed once per month, per patient. ThoroughCare will automatically generate the therapy-specific code, i.e. 98976, 98977, 98978, automatically at the end of the month for patients with 16+ days with readings.

Users will also be able to generate time-based claims for RTM, just like any other CMS program! For more information, see our RTM article here!
