How to customize and generate patient Care Plan Reports

Overview

ThoroughCare's Care Plan Summary Report provides patients, users, and other healthcare professionals an easy-to-read look at a patient's Care Plan and their documented progress, and allows users to make the report as simple or as comprehensive as the recipient requires.

Generating Care Plan Summary Reports

To generate an individual patient's Care Plan report, a user will navigate to the patient's Care Management screen (the selected program does not affect the report generation). From there, the user will click the blue 'Print / Preview' icon to populate the 'Select Report Details' modal.

From the modal, the user can pick and choose what information should be pulled into the report and what information is unneeded.

Here are some helpful tips for users when making their report selections:

  • The Date Range field determines what information is pulled into the report so that users can limit the data shown if the report is being generated for an up-to-date record or historical purposes.

  • The Select Template dropdown displays the practice's list of custom report templates, which, when selected, automatically mark the report selections based off the template's settings. To learn more about this functionality, please review the Report Templates section below.

  • The individual Programs options (CCM, BHI, CCO, RPM) will determine what programs the additional selections are populated from. Users can select one or more programs, depending on their needs.

    Note: Only practice-enabled programs will be displayed.

  • Each individual report option contains a hover tooltip that provides an additional explanation of the information that option will populate to the report.

  • The Custom Assessment dropdown will display all finalized client assessments performed on that patient. When selecting one or more assessments, the record will be included with the Care Plan Summary Report.

    Note: This dropdown will not be displayed if the user does not have any applicable finalized assessments.

  • The Select Text Size dropdown allows users to increase the font size on the report for recipients who would benefit from the larger print.

  • The Open Preview button populates a PDF view of the report in the user's browser, from which the user can print, email, download, and save as needed.

  • The Download PDF button automatically downloads the report into a PDF file without any additional steps.

  • A user's previous selections will be automatically displayed the next time they populate the modal.

Once generated, the report will be formatted as seen below. Please note that the Patient and Practice Demographic information, as well as the patient's 'Care Team' section will always be included in every report record.

Generating Bulk Care Plan Summary Reports

Outside of an individual patient's chart, users can also generate one or more patient Care Plan Summary Reports from any of the program-specific worklists. To generate a bulk report file, a user can select one or more rows on a worklist, click the 'Bulk Actions' button, and select the 'Care Plan Report' option.

This will populate the 'Care Plan Report' modal that operates identically to the 'Select Report Details' modal described above.

Note: The 'Custom Assessments' dropdown will always be shown and will include the full list of available client assessments. There is no need for concern if an assessment type is chosen when a selected patient has not had that assessment performed.

Once all necessary options are selected, the user can click the 'Generate' button to download all generated reports into one file.

Saving Care Plan Summary Reports

Whenever a Care Plan Summary Report is generated through a program worklist's bulk action or an individual chart, a copy of that report is automatically saved to the patient's Documents tab and is filtered under the 'Care Plan Report' document type.

These reports can be reviewed and downloaded through the Action column's 'View' function. They can also be removed from the Documents tab entirely through the Action column's 'Delete' function.
