Overview

Remote Therapeutic Monitoring (RTM) services allow healthcare providers to remotely monitor and manage patients using medical devices that collect "non-physiologic data" such as therapy adherence, therapy response, and musculoskeletal or respiratory status. These services help improve patient engagement, support better treatment outcomes, and provide reimbursement opportunities for the time spent monitoring and managing care.

Enrollment & Consent

Once RTM is enabled, users can enroll a patient by navigating to the patient’s page and selecting the "Care Management" tab. This will display the available program tabs for the practice.

From here, the "Enroll" button can be used to add the patient to the RTM program, allowing users to log time and create a Care Plan for that patient. Once enrolled, an "RTM Consent" flag will automatically be added to the patient’s record.

Note: RTM does not currently have a dedicated dashboard or worklist in ThoroughCare. Instead, the "RTM Consent" flag can be used to filter and locate RTM patients within the system.

How to Assign a Device to RTM

When adding a device to a patient, you’ll now see an option to select which program the device should be assigned to. If RTM is chosen, an additional dropdown will appear at the bottom of the modal. From here, you can select the appropriate claim type based on the type of monitoring the device provides.

While any ThoroughCare-supported device can be associated with the RTM program, the most notable is the Tenovi Pillbox. This device allows the care team to track medication adherence, a non-physiological class of data that is useful in the RTM program. Please see our Tenovi Pillbox article to learn more.

Billing & Claims

ThoroughCare supports the following "time-based" RTM CPT codes:

  • 98980 - Initial 20 minutes of RTM treatment management services per calendar month, requiring at least one interactive communication with the patient/caregiver during the month.

  • 98981 - Each additional 20 minutes of RTM treatment management services per calendar month, requiring at least one interactive communication with the patient/caregiver during the month.​

  • 98975 - Initial setup and patient education (requires 16 days with readings)

  • 98976 - Respiratory monitoring (requires 16 days with readings)

  • 98977 - Musculoskeletal monitoring (requires 16 days with readings)

  • 98978 - Cognitive behavioral therapy monitoring (requires 16 days with readings)

Note: For RTM claims to be submitted in ThoroughCare, a successful interaction must be logged for the patient during the month.
