ThoroughCare has a new integration with Health Gorilla. Health Gorilla provides the ability to access patient data by a master patient index, data repository, that can deliver patient data. Health Gorilla provides the ability to seamlessly share health data and aggregate each patient's entire clinical history in one place. ThroughCare's integration with Health Gorilla provides an ability to view patients' clinical data, ADT messages, and Consolidated Clinical Data Architecture (CCDA) through our platform.

With Health Gorilla, Case Managers are able to pull relevant health information including medications, allergies, and hospitalizations from a database into ThoroughCare. This allows Case managers to easily go over medications with patients, compare patient reported medications to what is actually being filled, and monitor for any new medications the patient may be taking. All steps allow for case managers to seamlessly perform a comprehensive medication reconciliation for their patients. In addition to medications, Health Gorilla can also send notifications if a patient is hospitalized or discharged. Case Managers can utilize this feature to reach patients with recent hospitalizations and ensure they have everything they need at discharge such as a follow up appointment, home health care, or an understanding of discharge instructions. All are important to reduce the risk of rehospitalization and improve patient outcomes.

Health Gorilla CCDA

When a patient is found in Health Gorilla, the user will see a CCDA button in blue in ThoroughCare, to denote that the patient was found. The CCDA button is functional where as the user can preview the "CONTINUITY OF CARE DOCUMENT" with data that is stored in the networks.

Once you select the CCDA button, there is a brief loading period that will display on your screen prior to the CCDA information displaying. Once the information loads it will display on your screen and you have the ability to print the document as well out of ThoroughCare.

Note: The data in the CCDA document will not all be parsed out and automatically added throughout the patient's chart in ThoroughCare

Note: The CCDA document shows data that is pulled from the networks [Commonwell® /Care Quality® ] that serve as a data collection for several Health Information Exchanges across the United States.

Clinical Data Points

Through the ThoroughCare integration; the system provides users with the clinical data parsed out (when applicable). This data is updated in ThoroughCare every 14 days from the networks due to auditing regulations that must be followed. The data is only updated every 14 days on ACTIVE patients. If a patient is inactivated for a certain period of time and becomes active; a time log update or product update will update the patient date automatically.

Allergies

Allergies is a clinical data point that is imported from Health Gorilla to ThoroughCare.

Medical Equipment

Medical Equipment is a clinical data point that is imported from Health Gorilla to ThoroughCare.

Note: If the user selects "edit" they will be able to see the day it was started.

Social History

Social History is a clinical data point that is imported from Health Gorilla to ThoroughCare.

Note: With future development cycles, we will add more clinical data points to continue adding value through this integration.

Note: For customers who solely use AWV and TCM the patient clinical data updates will be done when the visit is started and will not follow the 14 day interval as mentioned above.

Source of Clinical Data

The clinical data that is parsed into ThoroughCare is conveniently labeled in as "clinical_network." This will help differentiate clinical data from a user manually entering the information or if it was pulled from Health Gorilla.

ADT Messages:

The integration between ThoroughCare and the Commonwell® will allow for ADT HL7 messages to instantly transfer across to ThoroughCare and into the patients' chart.

Note: An ENS notification is available through email as well with a submitted list of users. Each individual practice will need to submit this request through account management when their integration is turned on.

Users will be able to see the individual message details under the Transitions tab by clicking on the "Summary" button under actions in the Transitions table.

Getting your Practice setup with ThoroughCare and Health Gorilla

Your organization needs to provide an NPI and its associated location name and location address; also provider organization and practice name.

You will need a point person to initially test your production data and validate data points when you activate this integration.
