Subscribing to the Health Data Exchange

Information about what information is transmitted via the Health Data Exchange and how patient subscription functions.

Using ThoroughCare's Health Data Exchange integration is a convenient and reliable way to ensure patient data is up to date. Subscribing a patient to the clinical network initiates a reciprocal flow of ADTs, conditions, medications, lab results, and more. Please speak with your ThoroughCare account representative for more information about enabling Health Data Exchange for your practice!

Subscribing a Patient

A few pieces of data are required to subscribe a patient to the health data exchange: provider NPI, patient name and DOB, and patient address (including city, state, and zip code). If your ThoroughCare practice is integrated with an EHR, many of your providers may already have their NPI numbers attached to their profile. If not, the ThoroughCare team can add these for you.

If the patient who is being subscribed has all of this required information, ThoroughCare will automatically attempt to subscribe the patient to the health data exchange as soon as the patient page is accessed. To confirm the subscription attempt, a new field will appear next to the patient's name on their profile saying "Subscribed: No" with a loading spinner.

As soon as the patient is subscribed, the "No" will switch to a "Yes". This may take a few moments.

If the patient is successfully subscribed, users will begin to see patient conditions (ICD-10s), ADTs, medications, vitals, and lab results added to the patient's profile in ThoroughCare if they exist in the clinical network.

Patients are subscribed for 90 days from the first date they are accessed. If the patient is accessed again within that 90-day time frame, the 90 days will reset. If the patient is not accessed again after 90 days, they will be automatically unsubscribed from the network.

Available Data

As ThoroughCare enhances the health data exchange integration, more data will be retrieved from the network. For now, the data that is returned include ADTs, conditions, medications, lab results, and vitals. Please note that the data flow is reciprocal - any information on the patient's profile in ThoroughCare will also flow back to the clinical network.

When an ADT is received for a patient from the clinical network, it will populate both on the "Transitions" tab and as a banner next to the patient's name.

Any data that has a section for "Source" will show "clinical_network", which will indicate that it came from the clinical network.
