How to Navigate ThoroughCare's Assessments Tab

Overview

ThoroughCare offers a number of assessments that are available to practices to complete on behalf of, or by their patients, per each patient's individual needs. These assessments are located in each patient's 'Assessments' tab and can be accessed at any time.

Assessment Types

ThoroughCare offers both standardized and custom assessment options. The following assessments are available in all practices:

  • Alcohol (CAGE): A 4-question tool used to screen for Alcohol Use Disorder.

  • Anxiety (GAD-7): A 7-question tool used to screen for Generalized Anxiety Disorder and assess symptom severity.

  • Depression (PHQ-9): A 9-question tool used to screen for Depression and assess symptom severity.

  • Health Risk Assessment: A questionnaire that assesses areas such as medical, social, and mental health history, lifestyle factors, and home safety to identify health risks and preventive care needs. Note: This assessment is also available through the Wellness tab when completing an AWV visit.

  • HRSN Screening: The Health-Related Social Needs Screening is a tool that assesses social needs such as housing instability, food insecurity, and transportation challenges to help identify and address barriers to overall health.

  • Mood Disorders (MDQ): A tool used to screen for Bipolar Disorder.

  • Substance Abuse (DAST-10): A tool used to screen for drug use and evaluate the severity of use.

If your practice would like to utilize any assessments that are not listed above, ThoroughCare also offers practices the option to create custom assessments that will operate similarly to our default options. If you would like to include additional assessments in your workflow, please reach out to a ThoroughCare representative to discuss further.

Performing an Assessment

To start any assessment in ThoroughCare, a clinical user will navigate to the intended patient's 'Assessments' tab, where all available assessments are listed in alphabetical order.

To search for the intended assessment option, users can type the assessment name in the table's 'Search' field or sort through the assessments by alphabetical order, status, by the date the assessment was last finalized, or by the user who last started the assessment. Once located, a user can begin the assessment and complete the questions within ThoroughCare, or they can send the assessment directly to the patient through text or email.

Performing an Assessment as a User

If a user is completing the assessment on behalf of a patient, they will begin the assessment by clicking the 'Start' button located in the 'Assessments' table's 'Actions' column.

If the user is not able to complete the assessment, they can click the 'Finish Later' button located at the top right of each assessment to save their responses and complete at a later time.

The assessment's status will update to 'In Progress', and the 'Start' button will update to 'Finish'. To go back and complete the assessment, any clinical user can click the 'Finish' button at any time.

Once the user completes the assessment and clicks either the 'Finalize' or 'Finalize Incomplete' button (depending on whether all questions have an entered response), the assessment's status will be updated to 'Finalized', the date and timestamp related to when the assessment was finalized will be populated, and the user who finalized the assessment will be displayed. At that point, the assessment will be available to start again at any time.

Sending an Assessment to a Patient

ThoroughCare also allows users to send assessments directly to patients to complete on their own through a link sent through either email or text message, and all responses will be immediately available within the patient's record. To send an assessment to a patient, click on the assessment's 'Send to Patient' button and select either the 'Send via Email' or 'Send via Text' option.

Note: If the 'Send via Email' option is disabled, the patient does not have a valid email address listed in their 'Patient Info' tab. Once it is added, the action can be completed successfully. If the 'Send via Text' option is disabled, either the practice or the patient is not set up to send/receive text messages through ThoroughCare's Messages module. Once the preferred action is selected, a confirmation modal will display where the user can review the information and opt to send the assessment or close the modal without sending.

Receiving an Assessment as a Patient

Once the patient receives their message and clicks on the 'Complete Your Medical Assessment' link, they will be directed to a welcome screen that will require the patient to accurately enter their date of birth to confirm their identity.

Once entered, the patient will be navigated to their assessment.

Note: Patients will not have the 'Finish Later' option available to users.

After the patient finalizes their assessment, it will be immediately available in the patient's 'Assessment' tab, and the assessment's status will update to 'Finalized', the date and timestamp related to when the assessment was finalized will be populated, and the user who initially sent the assessment will be displayed. At that point, the assessment will be available to start again at any time.
