Using ThoroughCare's ADT feed integration is a convenient and reliable way to ensure patient data is up to date. Subscribing a patient to the ADT feed will result in both a historical data pull of ADT data and a subscription to a real-time feed. Please speak with your ThoroughCare account representative for more information about enabling the ADT feed for your practice!
Subscribing a Patient
ThoroughCare offers two subscription options for this integration: only subscribe patients who are active in a program, or subscribe all patients in your ThoroughCare practice.
A few pieces of data are required to subscribe a patient to the health data exchange: provider NPI, patient name and DOB, patient address (including city, state, and zip code), and a patient phone number. If your ThoroughCare practice is integrated with an EHR, many of your providers may already have their NPI numbers attached to their profiles. If not, the ThoroughCare team can add these for you.
If the patient who is being subscribed has all of this required information, ThoroughCare will automatically attempt to subscribe the patient to the ADT feed overnight. If the patient has been successfully subscribed, the following icon will appear on the patient's page:
If the patient is missing any required data points, a system-generated flag titled 'ADT Subscription Failure' will appear on the patient:
When the patient is updated and all required information is populated, the flag will be automatically removed, and the patient will be subscribed to the ADT feed overnight.
NOTE: After a patient has been successfully subscribed to the ADT feed, it may take up to 24 hours for the patient to be registered in the ADT system. Please wait at least 24 hours before contacting the support team if you have not yet received your data.
ADT Data in ThoroughCare
When the patient is subscribed to the ADT feed, ThoroughCare will receive a one-time data file of the patient's history. This data will be processed and stored in ThoroughCare for a historical view of the patient's journey. After the historical data is processed, ThoroughCare will begin receiving real-time ADTs for further events.
When an ADT is received for a patient from the clinical network, it will populate both on the "Transitions" tab and as a banner next to the patient's name.
The ADT information will also populate in the patient's Encounters tab. Populating the information in this section of ThoroughCare enables greater visibility into population health through the analytics platform, especially in sheets such as the Stratification Dashboard and Report.
ADT Notifications in ThoroughCare
ThoroughCare can send users notifications when an admission or a discharge is received by the ADT feed. To receive these notifications, users must enable the 'New Patient and Result Imports' notification as shown below.
When an ADT is received for a patient via the feed, ThoroughCare will notify the patient's primary care manager if that user has the above notification enabled. The user will receive a notification for both the admission and the discharge event. The notifications have a priority of 'High'.
