Overview

ThoroughCare’s Automated Risk Levels feature streamlines how practices identify and manage patient risk levels using a data-driven algorithm. Provided through a practice setting, this feature activates risk-related functionality within patient charts and any relevant program-specific worklists. ThoroughCare will automatically assign a Low, Medium, or High risk level to each patient based on their documented medical data.

These levels are determined through a risk score algorithm based on the American Academy of Family Physician (AAFP) risk-stratified care management rubric.

Note: Once this setting is enabled, ThoroughCare’s default custom Risk Levels will be removed from the practice and replaced with the appropriate automated counterpart. Please reach out to your ThoroughCare representative to discuss making this change.

Automated Risk Levels

At the time that the practice setting is enabled, a risk calculation will be automatically performed for every patient in the practice, and then again any time a change is made to an area of the patient’s record that is considered by the algorithm.

Note: After the setting is enabled, it will take up to 24 hours for the calculations to run for all patients in the practice. If patient records are added or edited during that time, their risk will be calculated in real time.

Once the appropriate risk level is assigned it will populate in the patient banner located at the top of their chart.

Risk Level Categories

ThoroughCare's Automated Risk Level algorithm is comprised of five categories that consider different areas of documentation to calculate, not only an overall risk level, but individual risk levels as well. Those categories include:

Age

Evaluates the patient's current age

Utilizations

Evaluates the patient's use of the following medical resources by looking at the following:

  • Their current number of medications

  • Their annual number of emergency visits

  • Their annual number of inpatient hospitalizations

  • Their annual number of PCP visits

Chronic Disease

Evaluates the patient's potential need for ongoing management by looking at their current BMI and their number of chronic disease diagnoses

Behavioral & Mental Health

Evaluates the patient's ability to manage their own health by looking at the following:

  • Their history and current relationship with alcohol, illegal drugs, and prescription drugs

  • Their history and current relationship with tobacco products

  • Their familial and personal experience with mental health conditions

Social Determinants of Health

Evaluates the patient’s life circumstances outside of clinical settings that may affect their ability to access care, manage health, and maintain stability

‘Needs Review’

If there is pertinent data missing at the time of calculation, ThoroughCare calculates the risk using the documentation available, and will populate a ‘Needs Review’ flag next to that estimated level.

To review and update the missing data:

  1. Click ‘Needs Review’ to populate the ‘Risk Score Missing Info’ modal. This modal will display the sections of the patient’s risk score that have not yet been documented.

2. Click the ‘Start Risk Assessment’ button to populate the ‘Risk Score Assessment’ which will only populate the specific sections and fields required to complete the patient’s risk score calculation.

3. Complete the assessment questions.

4. Click the ‘Finalize’.

Note: If you are unable to complete all questions at once, you can click ‘Finish Later’.

Once the assessment has been finalized or saved, it will be accessible through the patient’s ‘Assessments’ tab.

Patient Risk Score Report

Each time a patient's a patient's Risk Level is updated, either either overall or in an individual categorey, a copy of their current levels will be automatically generated and saved in the patient's 'Documents' tab. To filter for these historical records, search 'Risk Score' in the 'Type' dropdown.
