Feature of the Month

The new Care Management Header gives care teams a clear, at-a-glance view of a patient’s care activity across all programs. ThoroughCare’s initial header release introduces three key sections designed to surface the most important information upfront:

  • Program Status - A view of all available care management programs and the patient’s current status in each.

    • Includes functionality to update the patient’s status directly through the Program Status header section.

  • Care Plan Progress - A chart that reflects the patient’s overall care management progress by visualizing the amount of ‘In Progress’ vs. ‘Completed’ care plan items.

  • Patient Interactions - A view of the patient’s most recent successful interaction and a list of upcoming, scheduled interactions.

    • Includes functionality to schedule, edit, and log interactions directly through the Patient Interactions header section.

Together, these enhancements help care teams quickly understand where a patient stands and what needs attention without digging through multiple screens.

New Features

‘Finalize Incomplete’ Assessment Practice Setting*

A new practice setting that allows practices to choose whether or not they want their users to have the ‘Finalize Incomplete’ option when completing patient assessments.

Glucose Management Indicator (GMI) Enabled Measurement*

A new Glucose Management Indicator (GMI) measurement option is now available, which allows care teams to view and track a patient’s GMI based on continuous glucose monitoring data.

System Enhancements

The APCM Enrollment & Consent process will now be completed through the Care Management tab and will no longer require the manual effort of adding the ‘APCM Consent’ patient flag. Current APCM patient enrollments will be updated automatically and will reflect the date they were associated with the ‘APCM Consent’ patient flag.

Resolved Issues

Patient Chart

  • Date picker fields no longer update the start date’s selected year when the end date is set to a different year

  • Numerical assessment questions now restrict entries that fall outside of the designated minimum and maximum values

  • When updating the ‘Visited at’ field in a TCM record from a previous year, the Communication Overview’s ‘Face-to-Face Visit’ section now reflects the accurate status

  • Users are now successfully navigated away from their current location when they utilize the ‘Click here to navigate to the new patient’ option after transferring a patient to a new practice

  • Medication notes are now reflecting the expected date when making and addendum

  • The ‘Patient will maintain a blood pressure of > (systolic)/(diastolic) until (date)’ SMART Goal can now be updated successfully

Worklists

  • Patients with an asterisk (*) in their EHR ID are no longer excluded when generating bulk Care Plan Reports

  • Columns that can be sorted alphabetically will now do so in the expected order

  • Users will no longer see the ‘CoCM’ global patient flag in their filters

  • When searching for patients with 10+ ‘Days with Readings’ from the RPM Worklist, the filter now returns all expected patients

  • The Billing & Claims Worklist Bulk Actions dropdown is now always displaying ‘Submit All’ instead of ‘Mark all as Submitted’ for customers with a billing integration

  • The TCM Worklist’s ‘Next Deadline’ column is now successfully updating when a patient’s TCM complexity is changed

  • Users can now successfully filter for Patient Flags that contain a comma (,) in the name

ThoroughCare Management

  • Call Purposes can now be associated with ‘All’ programs without multiple attempts

  • Any updates to a practice’s ‘Reading Alerts’ range settings through the Customer Management screen are now successfully saved when utilizing the ‘Set for All Patients’ function

  • Removing a user’s ‘Default User Status’ from their user profile no longer results in that user losing access to their assigned practices

  • The Customer Dashboard’s ‘# of Patients’ column now reflects the correct patient count for each practice

Analytics

  • The Wellness > HRA Details sheet is now accurately reflecting when an ACP is completed through the Health Risk Assessment

  • The Program Worklists’ > Risk Level column is now accurately reflecting the patient Risk Level

Integrations/API

  • Medication prescribers are now showing accurately for Elation customers

  • Improved AdvancedMD patient updates so demographic and clinical data sync successfully without blocking the update process

  • Patients are no longer being unsubscribed from Bamboo when a roster is not successfully uploaded

  • Bamboo rosters are no longer failing due to long running queries

  • Users will no longer receive a 500 error when deleting a reading that has been populated to the Device Readings Queue

  • AdvancedMD customers can now successfully retrieve patient Internal IDs through the ‘Updated from EHR’ function, allowing related claims to be successfully submitted to the EHR

  • Practices are no longer receiving unexpected updates from the clinical network for their patients

  • All expected users are now available in the GET/Practitioners response
