Overview

ThoroughCare now integrates with the Tenovi passive monitoring device, XK Essence, enabling real-time collection of patient data at any time without requiring manual input. These connected devices automatically capture and transmit vital measurements, including sleep duration, pulse, heart rate POC and respiratory rate POC, providing care teams with continuous insight into patient health, adherence and potential changes in condition.

The following are benefits of incorporating XK Essence when monitoring a patient’s health:

    • By tracking these vitals more closely, care managers can monitor progress, identify subtle changes in conditions, and help patients stay aligned with their progress.

Adding an XK Essence Device

To add an XK Essence device, navigate to the patient chart's 'Devices' tab and click the 'Add a Device' button to access the 'Add a new Device' modal. Once there, select the “XK Essence” device from the Device Type dropdown, and enter the Gateway ID in the “Device ID” section. Once all necessary fields are entered, click 'Save'.

After the device is successfully added, the 'Enabled Measurements' table will automatically reflect the associated measurements and links to the XK device.

XK Essence Device Readings

POC Measurements

The Pulse POC and Respiratory Rate POC are exclusive to the XK Essence device and represent proprietary passive monitoring capabilities that deliver continuous, contactless heart rate and respiratory rate measurements unique to this integration. Median Respiratory Rate and Median Pulse recordings are also available and can be selected via the “Enabled Measurements” dropdown.

These unique Probability of Change (POC) scores help you compare not only what is “within normal limits” across populations, but also for each individual patient. These scores quantify how unusual a patient’s most recent readings, in this case pulse and respiratory rate readings, are compared to their own baselines. This way, clinicians can proactively:

  • Identify patients with significant changes

  • Prioritize care

  • Triage patients needing closer assessment

It is important that users note that these scores are based upon the previous 7 days of measurements, meaning at least seven days worth of data must be collected by the XK Essence device to look for deviations.

Additionally, these POC scores range from -100 to +100, with positive values indicating measurements above baseline and negative values indicating measures below baseline. For example, a Respiratory Rate POC of -85 indicates an 85% likelihood that resting respiratory rate is below baseline. On the other hand, a Pulse POC of +95 indicates a 95% likelihood that resting heart rate is above the baseline. Scores of more than 80 or less than -80 in particular should be considered a meaningful change and a warning sign that the patient requires clinical assessment.

These scores will not change when:

  • Range of normal vital signs are high

  • The number of samples are too low

  • Vital signs are not changing

  • There is no clear established baseline (e.g. minimal days of data)

  • The onset of the change is very acute/sudden (e.g., Pulmonary Embolism, drug overdose, sudden cardiac arrest)
