ThoroughCare now integrates with the Tenovi passive monitoring device, XK Essence, enabling real-time collection of patient data at any time without requiring manual input. These connected devices automatically capture and transmit vital measurements, including sleep duration, pulse, heart rate POC and respiratory rate POC, providing care teams with continuous insight into patient health, adherence and potential changes in condition.

New Features
Problem Card Configuration*
Administrative users can now customize suggested care plan items at the practice level. Through the new ‘Suggested CP Items’ management screen, admins can add and remove the default goals, barriers, and interventions that are available to check-off at the time of Care Plan creation.
Customizable Problem Selection
ThoroughCare now allows clinical problem cards to be tailored specifically to individual customers or practices. This update ensures that specialized clinical content is only accessible to authorized organizations, supporting proprietary workflows and personalized care delivery. Care Management Header Conclusion*
ThoroughCare has completed the Care Management Header with three new sections designed to centralize critical patient information and streamline clinical workflows.
Care Coordination: This section features two primary views, Care Team and Other Providers, enabling users to monitor both internal ThoroughCare staff and external care team members assigned to a patient. Users can conveniently review and edit these assignments directly from the header to ensure care team alignment.
Patient Vitals: This section provides immediate visibility into recent vital readings and streamlined access to each enabled measurement’s details.
Time Logs: This section displays monthly logged time for each program in which a patient is actively enrolled to support time-based programs. Care team members can compare current progress against established time goals, providing a clear visual indicator of claim status and required outreach.
ThoroughCare now has a dedicated ‘Medical Necessity’ text field, which allows care teams to document the clinical justification for program participation independently of enrollment status changes. These notes are stored with a versioned history and can be included in the Care Plan Report as needed.
ThoroughCare now offers a dedicated area within the Patient Info modal to document a caregiver’s contact information. It also allows users to select the caregiver as the ‘Preferred Phone, which will automatically be reflected in the Patient Header and other primary contact areas throughout the platform.
Other Providers Details Sheet
This new analytics view provides a comprehensive export of the patient providers table. It captures provider names, specialties, NPI numbers, and contact details, ensuring care teams have full visibility into the external care team members documented for their patients.
Demographics Details Analytics Sheet
The Demographics Details sheet now includes a new ‘Extended Care Team Details’ view that has been added to report on custom care team roles and the specific members assigned to them. Furthermore, a ‘Patient Created Date’ field to help track the patient's enrollment journey.
Transfer Patients via API
The ThoroughCare API now supports programmatic patient transfers between practices within the same customer group. This allows technically advanced organizations to perform high-volume patient relocations while preserving critical data like patient flags and risk levels.
System Enhancements
TC Compass - Smart Summary Assessment Data*
ThoroughCare has enhanced TC Compass by providing the AI model with direct access to patient assessment data. The AI now synthesizes completed assessment questions, answers, and scores alongside other clinical data points.
Medent Integration: Automated GMI Transmission*
ThoroughCare has enhanced its integration with Medent EHR to include the automated transmission of Glucose Management Indicator (GMI) values. This update ensures that physicians have immediate access to critical diabetic health trends directly within the EHR, facilitating more informed clinical decisions and long-term progress assessment.
Patient Header Phone Notes
The content of the existing "Phone Notes" text field from the Patient Info tab is now displayed directly in the Patient Header. Surfacing these notes ensures that critical contact instructions or patient preferences are immediately visible when making contact.
CCO Program Update*
ThoroughCare now enables organizations to customize the display names of Care Coordination (CCO) to more appropriately reflect the work being captured under that program. Administrative users can now opt to define both a short and long name, which will replace all instances of ‘Care Coordination’ and ‘CCO’ throughout the platform, including the Care Management screen, the program Dashboard and Worklist, etc. If you are interested in renaming your CCO program, please reach out to your ThoroughCare representative.
Patient Import Update*
ThoroughCare has enhanced the Patient Import CSV to support the removal of patient flags. This update allows organizations to manage patient statuses more efficiently by enabling bulk flag updates through a simple spreadsheet upload rather than manual one-by-one removals.
Calendar Report Analytics Updates
The Calendar Report has been updated to include a new "Other Interaction Details" sheet for communications with "N/A" interaction types. Additionally, the Call Scheduler navigation and filters have been renamed to "Patient Outreach Dashboard" and "Outcome Status" to better reflect current clinical workflows.
Wellness: AWV Details Analytics Sheet Update*
A new ‘Wellness Medicare ID’ column has been added to the AWV Details sheet. This update allows users to quickly identify patients who are missing documented Medicare IDs, enabling more targeted outreach to collect necessary information for upcoming wellness visits.
Device Readings Queue Analytics Sheet Updates*
The Device Readings Queue Analytics sheet has been enhanced with new data points to provide deeper insight into how device readings are managed and reviewed. Three new fields, First Assigned, Reading Created, and Reading Taken, offer greater visibility into when readings enter the queue and when they are first assigned to a user. For practices using Disposition workflows, the report now also includes Reading Disposition, helping teams better track outcomes and analyze how device readings are handled across their organization.
RiskAssessment API Update*
ThoroughCare has enhanced the RiskAssessment API response to include additional datetime data points, providing greater visibility into the timeline of each record. The response now includes the time the record was first assigned to a user (first-assigned-at), completed (completed-at), taken (taken-at), and snoozed (snoozed-at), as well as the total duration the record was snoozed (snoozed-minutes). These additions enable API users to perform more detailed analysis of Risk Assessment workflows and record lifecycles.
Map Units for Patient Readings
The "Units" field in the Devices and Readings analytics sheet has been updated to display human-readable labels (e.g., "mmHg", "kg") instead of technical system codes, ensuring consistent and clear data presentation across all reports.
Reformat CPT Code Descriptions
CPT code descriptions within analytics have been reformatted to standardized, clinical labels. Examples include "RPM Monthly Monitoring" and "Systolic < 130 mm Hg," ensuring that billing and claims reports align with industry-standard terminology.

Resolved Issues
Integrations/API
Athena patients can now be bulk imported successfully.
Practices with an eCW billing integration will no longer receive an error when attempting to submit claims to the EHR.
The GET/RiskAssessment is no longer returning a 500 error.
Lab Results are now successfully importing from Athena.
Billing
The ‘Recheck Monthly Billables’ function no longer causes duplication for claims that calculate the date of service every 30 days.
Claims generated prior to an EHR integration are now submitting successfully.
E/M claim types will now submit to EHRs successfully.
The issue that stopped 99445 and 99454 claims with DOS of 4/1from generating has now been resolved, and the claims have been successfully generated.
Calendar
All interaction modals are now displaying the ‘Programs’ dropdown field.
Patient Chart
Users will no longer be locked out of patient charts when it is not actively in use.
Custom fields are now shown in the expected order in the Patient Info tab.
For Care Plan item notes that should be associated with a different date than the one they were created on, the selected date is now displayed as expected.
Users can now update the patient contact information without issue.
Users will no longer experience an issue downloading the Care Plan Report when APCM Patient Consent is included.
Care Plans, such as Lyme Disease and Obstructive Uropathy, are now populating the expected, condition-related suggested items.
When logging a call through the Care Management tab’s ‘Call Status’ icon,the record will now successfully save when a custom outcome is selected.
Users associated with a permission set that is enabled for ‘Log Time’ and disabled for ‘Backdate time logged’ are now able to log time as expected.
The Transitions tab will no longer display duplicate transitions received from Bamboo.
Patient note date/timestamps are now reflecting the correct time the note was saved.
Patients transferred from a practice without automated risk levels to a practice with automated risk levels are now being evaluated and assigned a risk level as expected.
When searching for care plan items from a managed problem card, the ‘suggested’ items are now displayed in the dropdowns as expected.
Branching logic around the Health Risk Assessment question ‘Do you agree to participate in the Nutrition and Physical Activity Assessment?’ has been corrected.
Users can no longer remove the global ‘ADT Subscription Failure’ patient flag.
Patient Portal
The last blood pressure reading is now displayed in the correct Systolic / Diastolic format.
ThoroughCare Management
Customer Admin users can now update Reading Alert ranges for all patients successfully.
Practices can now be successfully enabled for the ‘Gaps in Care Automation’ practice setting.
Worklists
The RPM Worklist’s ‘Supplied Device’ column is now showing the correct status for all patients.
The Device Readings Queue is now removing readings after 30 days as expected.
Interact notes are now saving successfully when added through a worklist’s ‘Call Status’ column.
Updated Care Manager and Providers assignments are now correctly reflected in the worklist views.
Patients who are transferred from an existing practice to a newly enabled practice are now successfully populating in worklists.
When filtering for ‘Enrolled’ patients, the CCM Worklist is now returning all applicable patients.
Users
In the event that a patient has not reviewed a Healthwise document, enabled users will only be notified once.
Users enabled for two-way text messaging notifications will now receive those alerts as expected.
