How to Upload Patient Documents in to the Patient Chart

Navigate to the Patient View Page:

On the ThoroughCare Dashboard, search for the target patient that is to be enrolled by selecting the search bar at the top of the page. Users can search by patient First Name, Last Name, or EHR ID number.

On the Patient Search Results screen, select the target patient by clicking their name, EHR ID, or "View" button under the "Actions" column.

View the Patient's "Documents" Tab:

Once on the patient page, locate the "Documents" tab, and click to select.

When the Document section is visible, notice the document table as highlighted in the below screenshot.

Note: If this patient has not had any files uploaded this table should be empty.

Uploading a new Document:

To upload a new document click the "Upload New" button located in the top right of the "Documents" section as shown in the below screenshot.

Once clicked, an "Upload File" pop-up should appear in the middle of the patient view page.

To select a file from the user's computer, click within the light blue labeled "Drag your file(s) here to begin or click to browse", this will open your computer's file explorer.

Note: You can also drag files from your desktop into the blue area to add the file to the pop-up.

Once the computer's file explorer opens, locate the file that needs to be uploaded and click to select.

When the proper file is selected click the "Open" button, as shown in the below screenshot.

If the file has been correctly selected, the name of the file should appear within the light blue box as highlighted below.

Now, the user can select the "Document Type" from the corresponding dropdown box. click within this dropdown box to view the list of document type options.

Next, select the document description that best fits the file that is being uploaded. Click to select, the target document type.

Note: If none of the document types fit the document you are uploading, chose the "Other" option.

Once the document type is selected, check the "Date Range" to make sure the day in which the file was created/uploaded is correct. If the date needs changed a user can click within the "Date range" box to change it.

Note: ThoroughCare should automatically populate the current date within this field.

If the "Date Range" box is selected, a pop-up calendar should appear for the user. Click to select the target day in which the file was created/uploaded.

Note: Users' may only upload documents with past dates, as one may notice no future dates can be selected.

Once the "Upload File" pop-up has been completed, click the "Upload" button in the bottom right hand corner.

Note: If you click out of this pop-up before clicking the "Upload" button your selected file will not be added to the patient's chart.

If the file has been uploaded successfully the document title will appear in the "Documents" table, as shown below.

Once a file has been uploaded, a user can view or delete a file by clicking the corresponding buttons underneath the "Actions" column.

Sorting Uploaded Documents:

If a user ever needs to sort the document table to find specific document types, they can use the "Document Type" dropdown. Click to bring up the list of document types.

Click to select a document type to filter by.

Users may also filter documents by "Date Range", click within the "Date Range" box to bring up the pop-up calendar.

Once the calendar is visible, a user may select two dates that they wish to filter documents by.

Note: Only documents dated between the two chosen dates will be displayed.

Another Note: Make sure to click the "Apply" button once your dates are chosen, if you are to click out of the calendar pop-up before clicking the "Apply" button the patient's documents will not be sorted.
