How to add conditions to the programs offered in the ThoroughCare platform (CCM, CCO, BHI, RPM).

Viewing the List of Conditions

In ThoroughCare when a user navigates to adding a new condition(s) for a patient, they will be presented with an improved list of conditions. To be exact, there will be over 480 conditions available to be chosen from.

If a user is looking for a particular condition; they are either able to scroll through the list and find the conditions or they are able to start typing where it displays "select one to add..." and they can type out the condition that they are looking for.

If for any particular reason the condition is not available that the user is looking for; they are able to proceed with selecting "Other Behavioral Health Condition" or "Other Chronic Condition(s)" which will then allow them to free text an option in.

NOTE: With the condition enhancements, over 180 conditions will have assessments attached to them. These will be displayed with a small clipboard icon next to the condition name (see screenshot below). We plan to continue to enhance our conditions by continuing to add assessments to the conditions that do not have them as of today.

Conditions Reconciliation / Update Modal

If a patient has had one or several free texted conditions in the past, the ThoroughCare system will pop open a modal for Conditions Reconciliation / Updating the free texted medication. The purpose of this modal is to be able to make all the conditions that are added for the patient to be reportable in Analytics.

NOTE: ThoroughCare will only provide a "SUGGESTED" condition. It is still the user's responsibility to select or save the appropriate condition.

The modal will continue to display for the patient as the user returns to the patient show page after exiting out of that particular patient's chart. To prevent the modal from appearing; the user needs to either approve or select the correct condition to map to and the modal will disappear. If the patient has had multiple free texted conditions, the modal will continue to display until all of the conditions are appropriately assigned one from the list provided.

NOTE: The Conditions Reconciliation modal will continue to display every time a user enters into the patient show page for that particular patient.

Current vs. Past Conditions

In the ThoroughCare software there will be the ability to view patients' current and past conditions. Users who have the AWV (Annual Wellness Visits) program will also be able to see a change for the current and past conditions that a patient has.

In the AWV program; Users will notice a difference in the first question of the exam which will display the "Current Conditions" and the "Prior" or past conditions that the patient has had. The list will be able to be updated through the AWV program as well as in ThoroughCare in other programs.
