Providers and Care Managers may now add the Referring Provider to a patient’s file when either updating or creating a new patient. The Referring Provider information will then populate within the patient file under Care Team and in the Care Plan Summary Report.

Add Referring Provider: New Patient
From the Dashboard, click on the ‘Add New Patient’ button to bring up the Create Patient modal.
Navigate to the Provider and Care Team section to find the Referring Provider dropdown.
Select the correct provider and fill out any text fields with an asterisk as they are required to complete before saving. Hit the ‘Create Patient’ button at the bottom of the modal when finished.
Note: Only users with the "Physician" role can be selected as a patient's referring provider.
Add Referring Provider: Existing Patient
Navigate to the desired patient page and find the ‘Edit Patient’ in the bottom-right corner under the Patient Info tab.
This will bring up the Update Patient modal where the user can scroll down to the Provider and Care Team section where the Referring Provider can be selected.
Note: Only users with the "Physician" role can be selected as a patient's referring provider.
The selected Referring Provider will then be shown under the Patient Info tab in the Care Team section.
The selected Referring Provider will also be shown in Care Plan Summary Reports that are created for the patient.
