ThoroughCare / Application Release Notes - November 11, 2025
Features of the Month
Description:
To-Do Automations introduce a powerful new way for administrators to streamline clinical workflows in ThoroughCare. Admins now have access to a dedicated Automations page where they can configure rules based on specific triggers within the system, such as when a patient is assigned a care team member or when a patient completes an assessment. Once an automation is active, when the chosen trigger occurs, a To-Do is automatically created and assigned to the appropriate user(s) ensuring timely and consistent follow-up without manual intervention.
Benefits:
Standardized Workflows: Ensure critical actions (e.g., a welcome call after team assignment or review after an assessment) are consistently assigned and followed up on every time a specific event occurs.
Reduced Administrative Burden: Automatically create tasks, freeing up staff time spent on manual assignment and monitoring.
Improved Compliance & Timeliness: Guarantee prompt action on key clinical events, helping to maintain quality of care and program adherence.
eClinicalWorks Integration*
Description:
ThoroughCare now supports pulling data from eClinicalWorks! The data points we can currently import include patient demographics, patient conditions, patient medications, and importing users. This enhancement will allow users to seamlessly retrieve patient data from the EHR, keeping crucial clinical data up-to-date.
Benefits:
Reduced manual effort: By simply entering the patient’s EHR ID, patient information will be pulled from the EHR into ThoroughCare.
Clinical accuracy: Fewer human errors since data no longer needs to be entered manually into ThoroughCare.
New Features
Description:
The Cross Practice Device Readings Queue enhances how you monitor and manage patient device readings. Previously, readings could only be viewed and acted on within a single practice at a time. With this update, the Device Readings Queue now supports cross-practice management, allowing you to see and respond to critical and caution readings from all practices you have access to, all in one central location. The queue icon will now display a yellow indicator whenever a new reading is received from any of your practices, ensuring you never miss an important update.
Benefits:
Unified Monitoring: View and manage readings across all practices without switching contexts.
Improved Efficiency: Save time and streamline your workflow by addressing alerts from one centralized queue.
Description:
The CPT II Codes feature enables practices to automatically include blood pressure–related CPT II codes with their time-based RPM billing codes each month. Specifically, this feature supports codes 3074F (Systolic <130 mm Hg), 3075F (Systolic 130–139 mm Hg), 3077F (Systolic ≥140 mm Hg), 3078F (Diastolic <80 mm Hg), 3079F (Diastolic 80-89 mm Hg), and 3080F (Diastolic ≥90 mm Hg). These codes are determined based on the most recent blood pressure reading taken during the month and continuously update as new readings are received. This ensures that your monthly RPM billing reflects the most current patient data. This is a practice-level option, so please reach out if you’d like this feature enabled for your organization.
Benefits:
Automated CPT II Tracking: Automatically apply the correct BP CPT II codes to time-based RPM claims.
Real-Time Accuracy: Codes update dynamically as new readings are recorded throughout the month.
Enhanced Quality Reporting: Improve accuracy in care quality documentation and performance measures.
Streamlined Workflow: Reduce manual entry and ensure billing data stays current and complete.
Practice-Level Control: Enable this option only where it best supports your billing and reporting workflows.
Description:
Leveraging the power of custom fields already used for patient tracking, you can now include these custom fields directly on Care Plan Reports. A new option to "Additional Patient Info" allows practices to print any custom data they track, such as unique local requirements or specific consent details, ensuring reports are comprehensive and tailored to their needs.
Benefits:
Personalized Documentation: Ensure reports contain all necessary, practice-specific patient information.
Enhanced Reporting: Utilize custom data for a more complete and useful documentation output.
Description:
We've enhanced the flexibility of patient care by allowing you to edit SMART goals after they have been added to a patient's profile. You can now update key details, including the goal values and due dates, directly within the smart goal record. This capability supports a more dynamic and complete workflow for managing long-term patient objectives.
Benefits:
Adaptability: Easily adjust goals and timelines as a patient's condition or progress changes.
Workflow Completion: Supports continuous care planning by allowing real-time modification of objectives.
Delayed Reading Indicators*
Description:
We've improved the Device Readings Queue to help you quickly identify readings that were submitted late or delayed. Now, when viewing a reading's elapsed time tooltip in the queue, you will see both the Date/Time the reading was received and the Date/Time the reading was taken, allowing for direct comparison. Additionally, in notifications, the "Reading Taken" time will now display in bold whenever it differs from the time the reading was actually received.
Benefits:
Clear Visibility of Delays: Quickly determine if a reading was received significantly after it was taken.
Improved Data Accuracy: Helps in assessing the timeliness and context of critical patient data.
ThoroughCare AI (Powered by CareCo) Single Sign-On
Description:
ThoroughCare has implemented the ability for users to single sign-on into ThoroughCare AI (Powered by CareCo) from a patient’s page in ThoroughCare. When the practice option is enabled, and a patient has a preferred phone number set, the phone number will become a clickable hyperlink in the patient demographics banner. When a user with a CareCo account selects the hyperlink, the ThoroughCare AI (Powered by CareCo) app will launch in a new tab, log the user in, and display the selected patient’s profile!
Benefits:
Productivity: Reduce clicks and improve navigation between systems
Set and forget: No need to remember your CareCo password, as you will be authenticated from ThoroughCare
Support for RTM CPT Codes via Device API Resource
Description:
Previously, only RPM CPT codes (99453 and 99454) were supported in ThoroughCare. With the implementation of RTM in ThoroughCare, new device CPT codes (98975, 98976, 98977, 98978) were introduced. When using the device endpoints of the ThoroughCare API, users now have the option to select which CPT (RPM or RTM) codes should generate when criteria are met.
Benefits:
Automated workflows: Programmatically distribute devices and assign their appropriate CPT code
